The Health and Life Events: HALE Surveys help you explore types of adversity that may have influenced your current health. It can be validating, eye opening, supportive to learn the types of experiences that can shape health.
Your responses also help refine our understanding of the role of adversity in risk for chronic illness and other physical and emotional health conditions and complaints.
These surveys are for adults with any state of health, including if you are in good health, which provides important data for comparison and analysis. The HALE surveys can be found below along with a survey for mothers who have a child that either has type 1 diabetes or related antibodies, or who is in good health with no chronic health challenges.
After completing a survey you will receive an email with your score and links to download your PDF Report for more context about each question and to see graphs of results.
These surveys have been approved by an independent Institutional Review Board (IRB), which is an organization that reviews research involving human participants to ensure ethical conduct and participant protection.

APOEs – Adverse Pre-Onset Experiences

This adverse pre-onset experiences survey explores the role of adverse events in the period before the onset of chronic health conditions, which may affect risk as well as the timing of onset.
The survey asks about events such as injury, infections, loss, vaccines, hospitalizations and more. The survey aims to help better understand if, how and when such adverse events may have contributed to the onset.
The APOE survey includes a score for the number of different triggers you may have experienced before onset. A confirmation email provides information, links and resources along with a copy of your responses to the survey.
