1 Hour 1 Page Medical History Consult for Chronic Illness and Trauma can be 1 hour to 2 hours

One Page Medical History Consult – 1 to 2 Hours

Variable Price

Creating or Refining Your One Page Medical History

As a retired family doctor and somatic trauma therapist I can help you figure out what is most important to include in a shortened history to help your doctor help you.

When you have a chronic illness a short, succinct summary of your medical history on one page has many benefits. It can save time for your doctor (and therefore you), help the two of you identify the most important issues more easily and make plans more quickly. It may help build your relationship with your health professional.

A one page summary can also be your ally

  • when preparing for or attending an appointment
  • so you don’t have to recall or repeat your medical history anew
  • making prep for a medical appointment less stressful
  • Less info to repeat
  • Less to remember off the top of your head.
  • Help remember details, dates, sequences of events, test results, and more.

1 to 2 Hours One-on-One

This one-on-one consult with me offers help on how to create or refine your shortened medical history. I recommend a zoom session if you’d like to see my screen for tips and phone is fine too.

Benefits Include help consolidating

  • a complex medical history
  • data from different facilities
  • a long history of chronic illness(es) with a lot of records
  • refining what to include
  • accuracy in what and how you summarize

You can use our time together to build on what you’ve already created after reading the One Page Medical History Guide that you download below. If you are just starting, you can also use the downloadable worksheet provided below.

This consult can involve discussing where or how to start, how to further organize a summary you’ve already begun. This could involve what to include, how to determine what to include, how to shorten words, and more. You can also use this consult to identify what to consider for another side of your one pager, such as a page presenting more information specifically related to your chronic illness(es). Note that this consult will help you with next steps. Depending on the stage of an existing summary it may or may not lead to a final one page medical history.

Fee

My fee for a medical history consult is as follows. There is a discount for the second hour if you need that:

  • 1 hour is $300
  • 1.5 hours is $400
  • 2 hours is $500.

The amount is in your currency, payable in US dollars. If $300 USD is not within your financial means, or to pay a different amount based on international exchange rates or after discussing with me, see my tiered fee structure.

Medical History Documents to Download

The following documents are for you to download before our consultation. Read the Guide and use the Worksheet to begin your One Page HIstory:

  • Medical History Guide
  • Medical History Worksheet

Your Information

In contrast to psychotherapy, which I no longer offer, consults focus on information, context for symptoms, and tips. We will not work directly with physical or emotional symptoms in our consult.

Have we met or interacted before, such as in person or through my blog? Please tell or remind me.

A little about you

To help me orient so we can make the best of our time, please tell me a little about yourself. Describe the reason for your consult, including any chronic illnesses you’d like to discuss. What are the outcomes you desire? If you are a health professional, tell me your profession and specialty area (Psychotherapy? Bodyworker?…). If you are a student, include what you are training or aiming towards. Is there anything else you’d like me to know?

I will contact you by phone or send you a zoom link. You will receive the zoom link again about 30 minutes before your scheduled consult.
